An evidence-based tool will be used to measure the current programs, including workspace and integration methods, patient identification, clinical services, patient engagement, and practice workflow. Recommendations will be provided.

An interdisciplinary team will work closely with the project leader(s) to customize and implement a PCBH or CoCM integrated model. Customized design and implementation of the preferred integrated model.
